I don't have one, but it seems to me it'd be fairly straightforward to make: crochet a tube in the stitch of your choice, or crochet a rectangle that you then whipstitch into a tube. If you made it with a rectangle, you could make it double-layered and put some quilt batting in between for extra coziness.

I don't have a pattern, but I remember having a fuzzy, maybe it was fur?, white muff when I was very little. It's a nice choice. I like LaMorocha's suggestion.
I have seen a pattern, but I think it was in a knitting magazine from sometime between 30 years ago and now. Not very useful. Ah, but I found a knitting pattern on-line for a muff, so you can take the dimensions from it, http://knitty.com/ISSUEsummer06/PATTmuff.html And use the design idea - ribbing at both ends, with the ribbing tucked inside for a second layer.
